But not if /boot is on LVM and grub does not read LVM.  It will not know where / lives.

Sounds like a flaky install aggravated by cosmic rays. Probably related to the 3ware controller.

Do a fresh install and this time walk very slowly through the partition set up process. If the partition setup doesn't see the 3ware raid mirror as a single drive, you may need to work in stages or dig for a better driver to use during install.

Hmm. Skinny down the process with a simple install just onto the software raid drive mirrors and skip the 3ware until after the install (unless you need the anaconda.log process for replication to another system). Then use manual tools to get 3ware driver module loaded in and create the partition and mount point, etc.
