In the ~/.fluxbox/init file, I usually put it on the last line:

session.styleFile:	/home/dsj/.blackbox/styles/special/twisties

That's the style file.  From there you can call the background:

rootCommand:    bsetbg -f ~/.blackbox/backgrounds/twisties.jpg

or whatever your background image is.  Notice, most of my stylefiles
live over in my blackbox installation, so that way I don't need
duplicate styles for BB and FB.
